That dual wiring didn't disappear when cats moved onto our sofas. It's still running in the background of every cat in every home, all day, every day.
Every sound, every visitor, every change in routine, every shared space with another cat, your cat's nervous system is scanning it, all the time, for one answer: is this safe or not?
When the answer is consistently "yes," their system can rest. When the answer is uncertain even in small, everyday ways, their stress response stays switched on for longer than it should.
What Chronic Stress Actually Does Inside the Body
This is where it stops being about "behaviour" and starts being about physiology.
When a cat perceives a threat, their hypothalamic-pituitary-adrenal (HPA) axis activates and releases cortisol, the primary stress hormone. In short bursts, this is protective; it's designed for exactly this. The problem is what happens when that system doesn't get to switch off.
Prolonged cortisol elevation suppresses immune function, which is why chronically stressed cats can become more susceptible to infections, slower to heal, and more prone to flare-ups of conditions like feline herpesvirus. It also affects digestion and bladder function and can contribute to inflammatory conditions throughout the body, not just in the moment of stress, but cumulatively, over time.

It Starts With You: The Human-Cat Nervous System Link
Here's the part most guardians never hear: your cat's nervous system doesn't just respond to their environment. It responds to yours.
If you've ever felt "wired but tired" after a stressful week, heart racing, sleep disrupted, digestion off, patience gone... that's your own nervous system stuck in the same activated state we've been describing in cats. Chronic human stress runs on the same HPA axis, the same cortisol pathway, the same inability to fully switch off. The research base specifically for cats is newer and still growing, but the early picture points the same way: cats are highly tuned to human emotional and physiological signals, and a home where the guardian is chronically dysregulated is, by definition, a less predictable and less "safe-reading" environment for the cat living in it.

The Bladder Is the Clearest Example And the Best-Studied One

For decades, cats brought to the vet with urinary symptoms and no infection, no stones, no obvious cause were labelled with feline idiopathic cystitis (FIC) — "idiopathic" simply meaning "we don't know why." Researchers at Ohio State University, led by Dr Tony Buffington, spent years studying these cases and found something striking: the cats had smaller-than-normal adrenal glands and a nervous system that behaved differently under stress. FIC, they found, wasn't really a bladder problem in isolation; it was part of a much larger picture involving the nervous system, the adrenal glands, and the environment the cat lived in.
Buffington proposed the term "Pandora syndrome" to capture this, a condition where the bladder is simply where the stress shows up first, but rarely where it ends. Cats with Pandora syndrome often also show signs in the gut, skin, respiratory system, and behaviour.
Here's the part that matters most for you: when researchers treated these cats not with more medication, but with environmental changes designed to reduce day-to-day stress, symptoms improved by 75 to 80% in some studies simply from feeding at consistent times, keeping litter boxes in stable locations, and reducing the small daily uncertainties in the cat's world.
That's not a marginal effect. That's stress management outperforming what many pharmaceutical approaches manage alone.
What This Looks Like in a Real Home

A hair cortisol study measuring long-term stress in shelter cats found that cats living in enriched environments, with resources like scratching posts and hiding places, had cortisol levels almost half those of cats without them. Hair cortisol reflects stress accumulated over weeks, not a single startled moment, which is what makes this finding so relevant: consistent, small environmental changes appear to lower baseline stress over time, not just in the moment they're introduced.
Other research on hiding spaces, vertical territory, and predictable routines supports the same pattern: cats given more control over their environment, where to rest, where to observe from, when to expect food, show measurably lower stress indicators than cats without that control.
The through-line in all of this research is the same: predictability and a sense of control regulate the nervous system. Chaos and uncertainty, even low-grade and constant, keep it activated.
So What Does "Prevention" Actually Mean?
Prevention has always meant good food, vet checks, healthy weight. That hasn't changed.
What the research adds is this: prevention also means paying attention to what your cat's nervous system experiences every day, the predictability of their routine, the safety of their resting spaces, the number of daily moments where they're left to wonder "am I safe?"
